The Patient Centered Medical Home Collaborative members address critical community health needs.
The Patient Centered Medical Home Collaborative (PCMH Collaborative) members serve patients from throughout the region. Many of these individuals have serious and complex conditions, including some of the leading causes of disease and death for the community. By serving populations with these conditions, PCMH Collaborative members help strengthen the health of the community.
The chart reflects the chronic conditions treated by PCMH Collaborative members which relate to the health of the community overall. As shown:
- All 12 conditions treated by PCMH Collaborative members are priorities for community prevention and control based on Healthy People 2020, various health indicators, and assessments conducted in the region.
- Seven of the 12 conditions are among the leading causes of death in the region, and five are among the leading causes of preventable hospitalization in the region.
