Impact on Quality

Patient Centered Medical Home Collaborative members are committed to continuous improvement and accountability for performance.

Patient Centered Medical Home Collaborative members (PCMH Collaborative) are community based, safety net organizations that rely on community investment to sustain their work.  To ensure that value is delivered in return for this investment, each organization is committed to a program of continuous improvement and performance measurement.  The performance profile presents selected indicators that illustrate the reach, productivity, and quality of the PCMH Collaborative’s efforts.

To document ongoing quality improvement, the Patient Centered Medical Home Collaborative members monitor quality measures appropriate to each clinic against nationally reported standards for accredited Medicaid health plans. Quality measures include tobacco cessation, cervical cancer screening, blood pressure control, and diabetes control.

Quality Measurement Dashboard

Patient Centered Medical Home Collaborative members regularly collect data on a set of quality measures. Clinics review their performance compared to other clinics in the PCMH Collaborative and compare their quality performance to external standards. This practice is commonly called “benchmarking.” For this project, the best available benchmarks are for Medicaid health plans nationally. The chart below shows selected quality measures collected by the PCMH Collaborative members for the fourth quarter of 2015 (October – December 2015).

  • The first column lists the quality measure
  • The second column provides a description of the quality measure
  • The third column provides a description of the community health concerns addressed by this measure
  • The clinic’s performance on each quality measure is displayed within the colored tier where it ranks nationally. The goal for each clinic is to achieve at least Second Tier performance on each measure.
