The Patient Centered Medical Home Model of Care development is a continuous process of innovation in which clinical practices implement improvements to their care models. Patient Centered Medical Home Collaborative members report on innovations or ‘breakthroughs’ on a monthly basis. A breakthrough is defined as any change in process, policy, or practice that has positive results at the clinic. Typically, breakthroughs are lessons learned by addressing issues or problems at the clinic. Breakthroughs do not have to be large in scope; in fact, some seemingly small breakthroughs can have the largest impact on clinic practice and culture.
