The Chronic Care and Patient Centered Medical Home Models of Care

The Chronic Care Model, developed by the MacColl Institute, is one of four theoretical frameworks behind the Patient Centered Medical Home model of care. Although the Chronic Care Model and Patient Centered Medical Home models are not the same, there are concepts in each model that support elements of the other model.

Below is a crosswalk between the Chronic Care Model and Patient Centered Medical Home models. The purpose of this matrix is to illustrate where elements of the Chronic Care Model reinforce elements of the Patient Centered Medical Home model, and vice versa.
